Hazel Stevens (nee Straub), age 65, of Hammond, Indiana passed away August 25, 2017. She was born on April 27, 1952 in East Chicago, Indiana to Mary Straub. She was preceded in death by her parents, her brothers John and Edward and also her sister Marie. Hazel is survived by her loving husband Jerry, brother Jimmy (Joanne) Straub, step-children: Suzie (Jason) Adams and Adam (Kim) Stevens, grandchildren, nieces and nephews.
